how to re-seal old bottles?

Does anyone here have any experience on re-sealing old bottles of port? I would be very happy if someone would have any advice on how to put an additional protection on top of old and fragile corks. Especially if there are initial signs of seapage and I don't want to be forced to drink it right away. I have some old bottles bought at Berry Bros and they have put quite a large rubber/plastic cap on top, which I assume is for improved protection. Can you do this yourself? What equipment do you need?
